Does Risk Assessment Make a Difference? Results of Implementing the SAVRY in Juvenile Probation.

Abstract
An effective approach to reducing recidivism is, first, to identify a youth's risk of reoffending and then to match the intensity of interventions to that risk level. This pre-post quasi-experimental, prospective study compared 247 (pre) with 217 (post) adjudicated youths to examine the implementation of the Structured Assessment of Violence Risk in Youth (SAVRY) and its effects on case management practices in Louisiana's Caddo parish probation office. The results indicated that placement rates dropped by 50%, use of maximum levels of supervision dropped by almost 30%, and use of community services decreased except for high-risk youths, but only after the SAVRY was properly implemented. This shift towards more appropriate allocation of resources that are matched to risk level occurred without a significant increase in reoffending. The implications for implementation and for use of risk/needs assessment in juvenile probation are discussed. Copyright © 2012 John Wiley & Sons, Ltd.

Probation Officers' Perceptions of Youths' Risk of Reoffending and Use of Risk Assessment in Case Management.

Abstract
Juvenile probation officers (JPOs) are required to make numerous decisions about the case management of young offenders on a daily basis. This multi-site study examined JPOs' (N = 64) perceptions of the typical youth's risk of reoffending before implementation of a risk/needs assessment (RNA) tool, and their self-reported, case management decision-making after implementation of an RNA tool. Results indicated that JPOs tended to overestimate the likely base rates of reoffending while RNA tool estimates were more accurate. Further, most JPOs appeared to be making service referral and placement decisions commensurate with youths' risk levels, regardless of whether they claimed to use the RNA tool in their decisions. Variability in application of risk to case management practices was more a function of the probation office than of the specific JPO. Implications for use of risk assessment in juvenile probation are discussed. Copyright © 2012 John Wiley & Sons, Ltd.

As a senior corrections officer at the Federal Detention Center, Richard Spisak had a predilection for preying on inmates, using his position to coerce male inmates into having oral sex with him, federal prosecutors said.

He even went so far as to reorganize his office, strategically aligning filing cabinets and placing papers — including prison rules and regulations — on the windows to block the view of the chair that he told one inmate would be his "assigned chair."

Spisak, 33, of Warminster, was sentenced to 32 months in a federal lockup Thursday for forcing a male inmate identified as J.W. to perform oral sex on him in January 2010. U.S. District Judge Paul Diamond ordered that Spisak, who had been free under house arrest with electronic monitoring, be taken from the courtroom in handcuffs.


Diamond, who found that Spisak also had "inappropriate sexual contact" with two other male inmates, said the former correction officer's conduct was "appalling." The sentence was twice what the high end of the sentencing guidelines recommended.

Spisak pleaded guilty in March to one count of engaging in a sexual act with a ward.

J.W. testified that after he had been transferred to the FDC in 2009, Spisak saw him every day the corrections officer worked and repeatedly "made advances" on him and threatened him if he didn't cooperate with Spisak's sexual demands.

On Jan. 5, 2010, J.W. said Spisak took him into his office and said he would set off his body alarm and tell authorities J.W. tried to assault him unless he performed oral sex.

According to court testimony, J.W. spit onto his own T-shirt after Spisak ejaculated then covered it with his overalls. DNA tests subsequently matched the stained shirt to Spisak. More

The second part, Meanings, examines the central role of understanding and a shared first person perspective, both against attempts to reduce meaning to basic information-processing mechanisms and to explain away the difficulties of understanding psychopathology in recent models of delusion.

The third part, Facts, shows the importance of uncodified clinical judgements, both in assessing the validity of psychiatric taxonomy and in the application of Evidence Based Medicine. Despite advances in the codifaction of practice and operationalism of diagnosis, an element of judgement remains in the assessment both of what, at one level, is good evidence for diagnosis and treatment and what, at a higher level, is good evidence for the validity of classification overall.

Abstract
In mammals, olfactory bulb granule cells (GCs) are generated throughout life in the subventricular zone. GABAergic inputs onto newborn neurons likely regulate their maturation, but the details of this process remain still elusive. Here, we investigated the differentiation, synaptic integration, and survival of adult-born GCs when their afferent GABAergic inputs are challenged by conditional gene targeting. Migrating GC precursors were targeted with Cre-eGFP-expressing lentiviral vectors in mice with a floxed gene encoding the GABA(A) receptor α2-subunit (i.e., Gabra2). Ablation of the α2-subunit did not affect GC survival but dramatically delayed their maturation. We found a reduction in postsynaptic α2-subunit and gephyrin clusters accompanied by a decrease in the frequency and amplitude of GABAergic postsynaptic currents beginning ∼14 d post-injection (dpi). In addition, mutant cells exhibited altered dendritic branching and spine density. Spine loss appeared with mislocation of glutamatergic synapses on dendritic shafts and a reduction of spontaneous glutamatergic postsynaptic currents, underscoring the relevance of afferent GABAergic transmission for a proper synaptic integration of newborn GCs. To test the role of GABAergic signaling during much early stages of GC maturation, we used a genetic strategy to selectively inactivate Gabra2 in precursor cells of the subventricular zone. In these mice, labeling of newborn GCs with eGFP lentiviruses revealed similar morphological alterations as seen on delayed Gabra2 inactivation in migrating neuroblasts, with reduced dendritic branching and spine density at 7 dpi. Collectively, these results emphasize the critical role of GABAergic synaptic signaling for structural maturation of adult-born GCs and formation of glutamatergic synapses.

Abstract
PURPOSE: To examine the estimated rates of mental disorders and associated situational characteristics in people involved in nonfatal use of force incidents with police in VIC, Australia. METHODS: A random sample of 4,267 cases between 1995 and 2008 from a dedicated police Use of Force Register were linked with the state-wide public mental health database and a police contacts database. Rates of ICD 9 and ICD 10 mental disorders recorded on the public mental health database were examined, as well as rates of criminal offending and the characteristics of force used by both parties. RESULTS: More than a third of people on whom the police resorted to using force (n = 1,621, 38 %) had a history of mental disorder. Significant overrepresentations of the estimated prevalence of psychosis [12.5 %, OR = 9.03, 95 % CI (7.41, 11.01), p < 0.001] and schizophrenia [9.1 %, OR = 9.73, 95 % CI (7.59, 12.47) p < 0.001] were found. Those diagnosed with mental disorders were 1.52 times more likely to use or threaten to use weapons on police, even after taking into account age, sex, substances intoxication and violent behaviour [95 % CI (1.23, 1.91), p < 0.001]; however, they were no more likely to injure or be injured by police than those without a recorded history of mental disorder. There was a noted trend for police to use, or threaten to use, weapons on people with a history of psychosis (other than schizophrenia) [OR = 1.40, 95 % CI (1.11, 1.78), p = 0.005]. CONCLUSIONS: Psychoses and schizophrenia are dramatically overrepresented in cases where police resort to using force. Situational characteristics evident in the encounters are suggestive of a sub-group of people with mental disorders presenting with aggressive and otherwise problematic behaviours coupled with histories of criminal offending; this presents significant ongoing challenges for the police.
